Please include some days and times that would work for you within our business hours so we can more easily have options available for you to schedule your appointment. Please keep in mind we are not responsible for technical issues and do not troubleshoot during the Zoom meeting, you must make sure you have this up and running and ready to go prior to your scheduled appointment.

  • All results will be sent to you prior to your appointment so you will have it on hand for our review.
  • To find a lab that can draw your kit you can call the number on your Access Medical Box.

2. You must also be prompt and available at the time of your appointment. We do realize issues arise, so we will give you 10 minutes to join the zoom meeting (if chosen), if a phone call is chosen, we call one time at the time of the appointment. You will have a 10-minute window to return our call. After 10 minutes, if we have had no contact from you, your appointment will be cancelled. You will be responsible to call or email back to reschedule at your convenience. If arriving at your appointment late, this will not extend your appointment, you will get only the allotted time for your appointment with respect to our other scheduled visits.
